The "Preemie" part refers to the size of the hole at the top of the nipple and can be used for any aged infant. The smaller the hole, the slower the flow of the liquid through the nipple.
I needed to start my son on a bottle because, while I breast feed, I am going back to work and wanted to make sure he could transition to a bottle. My Medela pump came with 2 nipples that were level 1. The flow was too fast for him. He choked, coughed, and spit up a fair amount of the pumped milk. I found it also made him a lazy breast feeder - he got frustrated that the breast flow wasn't as fast and easy as the bottle. My lactation consultant recommended using the Preemie size nipple - and it turns out it works very well. The flow is not as fast; he doesn't get choked, and it's more similar to breast feeding.
The nipples are made of silicone, not rubber, and have held up well so far. The Dr Brown nipples fit into Medela, Evenflow, and Ameda bottle tops (they may fit into others but these are the only type of bottles that I have), so there is no need to purchase Dr Brown bottles if you aren't already using them.
My daughter is not a preemie nor are we breastfeeding. I purchased these because my daughter was doing a lot of gulping and choking when feeding. We used the Dr. Brown's bottles with our older daughter and had similar problems, she had colic and at the time I didn't know that Dr. Brown's had a preemie nipple. I wish I would have known! My daughter has slowed down when feeding, she still does some gulping but it's not as bad and she doesn't get choked like before. As a result of feeding slower, she has less gas and tummy troubles. If your little one is having similar issues, give these a try.
